The problem with weight lifting and resistance training–although both are effective ways to incorporate strength training in your exercise routine–is that a lot of exercises are easy to mess up, especially if you’re working with heavy weight.

If you’re not familiar with proper form for the exercises you’re performing, there’s a high risk for error. Even with seemingly simple exercises, like dumbbell bicep curls for example, there are lots of things that can go wrong.

Not only do mistakes like these make the exercise much less effective (obviously the goal of a bicep curl isn’t to exercise your shoulder muscles), but they can also lead to injury.

Injury is the enemy. Do you know what the most important part of any trainer’s job is? Injury prevention. No ifs, ands or buts. And even if you don’t have the means to work with a coach or trainer, it should still be your first priority, too. That means following proper exercise form.
After all, you have lofty fitness goals and you won’t be able to put in the work needed to accomplish them if you have to take time off from the gym.

I asked Anderson to share a list of some exercises that he thought could be improved upon. It includes everything from moves that are commonly executed improperly to popular gym machines that might not always be the best option. I also asked him to share upgrades for each; alternatives that he feels are more effective.

Four Must Know Facts About Clinton Wealth

If Republicans are looking for someone who is out of touch and does not reflect middle-class values at heart, they only need to look in the mirror.

These assaults from the right on the success of the Clintons only show how far removed from reality the Republican Party has become. Americans know the Clintons are a great success story. Neither Bill nor Hillary Clinton were born into substantial wealth, yet they have lived the American dream and dedicated their lives to ensuring that others can do the same.

As Hillary Clinton herself said, “All one has to do is look at my record going back to my time in college and law school to know not only where my heart is, but where my efforts have been.” So let’s look at the facts.

  • Throughout her Senate career, Hillary Clinton was a strong proponent of policies that would help middle-class families throughout America. She fought to increase the minimum wage, cosponsoring the Fair Minimum Wage Act and introducing the Standing with Minimum Wage Earners Act.
  • Hillary Clinton was also an early fighter for equal pay for equal work. She introduced the Paycheck Fairness Act in 2005 and 2007 to prevent employer retaliation against workers claiming wage discrimination. She was also a cosponsor of the Lilly Ledbetter Fair Pay Act, which would become the first law signed by President Obama.
  • Hillary Clinton’s work advocating for out-of-work Americans was “a case study of how legislative objectives can trump ideology,” according to the New York Times. She worked across the aisle “to help deliver added unemployment benefits to millions of Americans” and repeatedly cosponsored legislation to extend unemployment benefits for Americans out of work, including during the 2008 financial crisis.
  • Going back to her time in Arkansas, Hillary Clinton worked to expand access to early childhood education for low-income families, helping to give future generations the opportunity to move into the middle class. Going even further back, Hillary’s first job out of law school was working for the Children’s Defense Fund, whose mission is to “ensure a level playing field for all children.”

These are just four examples from Hillary Clinton’s record, but I think the point is clear. She has dedicated her entire life to working for middle class families. Hillary continues to fight for them today, with the Too Small to Fail Initiative, and will continue to fight long into the future.

Yes, the Clintons have been rewarded for their hard work. But they have given back so much, from charitable donations, to policies that help low-income families, to the hope and leadership they’ve shown millions of Americans. Bill is still the man from a place called Hope, and Hillary is still the eager and capable college student looking to make a difference in the world.

The right wing’s agenda is transparent; Republicans don’t have a strong record helping the middle class so they’ve turned to baseless attacks. They will likely continue these attacks through 2016, should Hillary decide to run. I say let them. Her record will speak for itself.

Improving Healthcare and Reducing Cost Through Clinical Data Analytics

Jim needs to have his knee replaced. He is a 52-year-old former long-distance runner who has recently picked up on CrossFit and often wears a Fitbit. To schedule the surgery, he goes online and coordinates his family’s calendar with the calendars of his surgeon and the pre- and post-operation nursing teams. He is asked a few personal questions: In the past year, did he ever have trouble covering co-payments? Does he have friends or family who can transport him to rehabilitation therapy? Does he have to climb stairs to reach his bedroom?

After a short wait he receives his Quality and Risk Report Card. The report card says he is an excellent candidate for total knee arthroplasty (TKA). However, his recent trip to the urologist for kidney stones and urinary tract infection (UTI) suggests prophylactic antibiotic therapy prior to the surgery to reduce the risk of surgical site infection. And, because of his lack of adherence to prior clinical treatment plans, a post-discharge care coordination team will be engaged before he leaves the hospital.

If all of this sounds worlds apart from the kind of surgery your father might have experienced, it is. I spoke with Randy Salisbury, chief marketing officer of Streamline Health Solutions, about how healthcare is rapidly moving from relatively subjective decision-making based on isolated incidents to more evidence-based medicine; becoming connected, predictive and personalized. The days of the trip to the doctor resulting in referrals followed by the same tests and questions repeated over and over are nearing an end. Patients are taking a more active role in their own care. With relevant historical information about the patient and the experiences of patients like them, care teams can avoid undesired outcomes such as surgical site infections, ongoing pain, stiffness and poor physical function. This can save thousands of dollars in unreimbursed follow-up care costs for hospitals and health systems and can help keep patient satisfaction high.

How is all this possible?

Although hospitals and health systems have long monitored financial and operational data, the difference comes with tracking clinical data. This includes information such as a patient’s diagnosis, treatments, prescriptions, lab tests and hospitalizations.

Clinical data is extracted from a variety of sources, including electronic health records, information exchanges, disease registries, and even personal health devices and direct patient surveys. Examining clinical data completes a 360-degree view, enabling providers to analyze their patient populations, understand which individuals need the most help and proactively reach out to give them the care they need. When intelligently applied, clinical analytics help not only support a hospital’s bottom line, but also improve patient outcomes and reduce avoidable re-admissions.

What’s behind the drive toward analytics?

For one, it’s you, me and everyone else who waits to get help for a medical condition until they are sweating and complaining of chest pain and shortness of breath. It is no secret that “reactive” healthcare is more costly than preventive care designed to keep patients out of high-cost settings such as the emergency room.

Encouraging “wellness,” in fact, lies at the heart of healthcare reform initiatives that move away from fee-for-service reimbursement and toward value-based, accountable care models that tie together cost and quality of care. For hospitals, this shift means that future success depends on the ability to use data and analytics to improve both financial and clinical performance.

How it works

In the past, evidence-based treatment has meant following well-studied care protocols. With increased access to clinical data and advances in computing power, however, predictive analytics can provide a new kind of evidence to drive action. Aggregating and analyzing information about a broad population of patients over extended periods of time can help care managers understand–and therefore facilitate–the root causes of good outcomes for individual patients and for patient populations.

Predictive modeling helps care managers identify patterns in a population’s health in order to prospectively determine individual risk scores. These scores can then be used to triage the work of a care team, allowing them to focus first on patients at highest risk. In the earlier example of Jim, the TKA patient, predictive analytics helped identify him as a high risk for surgical site infection–and therefore in need of prophylactic antibiotics–or it might identify him as being inexperienced with physical therapy, which is a critical component of post surgery success.

Predictive modeling, whether based on claims data, medical records or patient-supplied risk assessments, can segment individuals prior to service and address a myriad of concerns before they turn into problems. This can be done through:

  • preventive care and wellness activities
  • chronic disease management programs that teach patients how to care for themselves, such as when to take their medications
  • better care coordination, for example, making sure medical records are sent to a skilled nursing facility prior to a patient’s transfer from the hospital

Clinical analytics can quantify everything from patient outcomes to readmissions and emergency department visits, to wait times and utilization of high-cost services. This new level of insight and transparency is good for both clinical outcomes and for business. In addition to setting internal benchmarks to measure cost and quality performance, for instance, clinical analytics also can help an organization compare how well it stacks up against its competitors and use that data to negotiate better payer contracts. It gives hospitals the ability to quickly make strategic decisions to improve clinical quality, reduce costs, optimize resources and enhance their competitive positions.

The benefits to patients are equally strong. By providing a holistic understanding of patients, analytics ushers in a new era of personalized healthcare. Clinical analytics have the power to arm individuals with timely, relevant information to help them make good health choices. This 360-degree view helps doctors predict risk for disease and response to treatment, supporting better diagnoses, safer drug prescribing and more effective treatments. Over the years, both as a patient and in his role as an executive for a leading healthcare analytics solution provider, Randy has witnessed this evolution first hand.

Forward-looking hospitals have already begun to embrace clinical analytics–and that means their patients can enjoy a life well-examined. With that will come lower costs, reduced readmissions, improved outcomes, and a better overall patient experience.

Erykah Badu Tries To Kiss News Anchor During Live TV Broadcast

Erykah Badu is our new favorite prankster. The singer crashed a live TV news broadcast in a few fantastic ways. News anchor Mario Diaz was trying to report on Shia LaBeouf’s arrest for New York’s PIX 11, but was interrupted by Badu and her huge white hat. She made some lewd hand motions — sex jokes! — walked back and forth and tried to kiss Diaz. Like a true professional, he stiff-armed her and pushed her out of the frame.

Some people on Twitter thought the culprit looked like Badu, but she denied it at first. Luckily for the TV-watching world, the truth came out.

Later, Diaz and PIX 11 acknowledged Badu and all was right in the TV anchor videobomb world. Check out the full video below.

My Husband Left 'Out Of Nowhere' Right Before Our 10th Anniversary

Yesterday was my 10th wedding anniversary. I started planning our celebration of this day about six months ago, researching romantic getaways, making an appointment with a professional photographer to have boudoir pictures taken as a sexy anniversary present for my husband, putting heartfelt words to paper for a possible vow renewal ceremony. I wanted to make this day as special as I could for us. Ten years is a really big deal, right?

But all my planning came to a screeching halt about four months ago, when my husband informed me he was no longer happy in our marriage and had decided to rent a house outside of town. It came out of nowhere, and I was completely blindsided. Sure, we had been fighting a bit, but not for one moment did I believe our problems were enough to tear us apart. But the sad reality was, my husband already had one foot out the door, and I didn’t even realize it. While I was trying to plan a fun family weekend, he was opening his own bank account, purchasing home furnishings and rallying a few friends to help him move that weekend.

The days and weeks that followed were a blur, as I tried my best to make sense of what had happened and started to pick up the pieces. There were so many things to think about, and some days it was all I could do to get out of bed. You never really understand how devastating divorce is until it happens to you. I was trying to understand, trying to heal, and scared to death at the thought of being on my own. But as my mind was swirling, one thought kept running through my mind: How the hell am I going to get through our anniversary?

I imagined the worst –- spending the day wrapped up in our anniversary quilt, eating quart after quart of that wine ice cream I found on Pinterest, watching painfully sappy movies based on ridiculously romantic Nicholas Sparks novels while bawling my eyes out and convincing myself that I was doomed to be alone forever. Or poring over our wedding photo album as our wedding CD played softly in the background, remembering every detail of that beautiful day 10 years ago. I was fully prepared to throw myself the most pathetic pity party possible that day.

But then I thought about how far I have come in the last four months. I feel stronger every day. I may not ever understand what happened to us, but I do know in my heart that it is over, and that things will never be the same between us again. I am starting to feel optimistic about the future. And I realized that to sit around sobbing about the past would be a giant step backwards. So I decided to make this day my bitch.

I got up in the morning and selected my most upbeat playlist to accompany my morning routine. I hugged and kissed my children repeatedly as we got ready for our day. I dressed in my favorite outfit and headed to work. I kicked ass that day, earning several words of praise and encouragement from my boss. I called the photographer who had planned to take the boudoir photos and scheduled a family photo session just for me and my daughters. Hey, that deposit was nonrefundable.

I talked an old friend into meeting me for a couple of drinks after work. And then, later, I met the best group of friends a girl could ask for at our favorite restaurant for margaritas and Mexican food. God bless these girls. They have my back in a way that sometimes I feel I don’t deserve. They really came through for me, and I left the restaurant sore from laughing so much and feeling loved by people who accept me as I am and genuinely want the best for me.

And as I crawled into bed last night, exhausted but smiling, I realized that I had managed to give this dreaded day a whole new meaning. I had decided that perhaps it is time to start moving on, that I am the only one who can make me happy, and that even though one person had deemed me unworthy of his time and love, there are so many others out there who think I am pretty damn amazing. And that is what I am going to focus on from here on out.

Best of all, I don’t have to change all my passwords. A good friend reminded me that even though I will no longer mark the day I married my “best friend and soulmate,” this date will always have significance, because it was the beginning of a love and life together that brought my two beautiful children into the world. And for that, I will always be grateful.

For example, have you ever thought of grilling cheese? Not cheese sprinkled on top of bread, but actual cheese. It’s entirely possible, surprisingly easy, and doesn’t turn into a gooey mess, but soft and chewy instead. Choose thick slices of halloumi or cheese wheels with a thick rind.

Grilled vegetables are another underappreciated grilling gem. One of the best is lettuce, which can enhance any summer salad. Try using a head of Romaine lettuce or butter lettuce that has been cut in half and brushed with olive oil. It chars in minutes and is simply irresistible drizzled with ranch or buttermilk dressing.

KLM Under Fire For Offensive World Cup Tweet

Dutch airline KLM made a big social media no-no when it tweeted an image celebrating the Netherlands’ World Cup win.

Minutes after the Dutch team beat Mexico, the airline posted the following:

The picture contained an cartoonish picture of a man wearing a sombrero and a poncho. Twitter users found the tweet to be racist and offensive.

Mexican actor Gael Garcia Bernal, who has nearly 2 million Twitter followers, called out the airline:

The airline quickly deleted the post.

Obama Downplays Backlash Over Hillary Clinton's 'Dead Broke' Comments

While former Secretary of State Hillary Clinton has recently come under fire for comments about her personal wealth, President Barack Obama believes her comments won’t make a difference in the long run.

During an interview with ABC’s Diane Sawyer earlier this month, Clinton claimed she and her husband, former President Bill Clinton, were “dead broke” upon leaving the White House. After facing a lot of scrutiny for the comments given the couple’s very comfortable financial state, Clinton attempted to distance herself from “the truly well off,” saying voters don’t viewer as “part of the problem” of economic inequality.

Obama said the the scrutiny that followed her comments won’t mean much for the potential 2016 candidate and longtime public servant, saying her “track record on that speaks for itself” in an interview with ABC’s George Stephanopoulos that aired Sunday. The president was in Minneapolis, Minnesota at the time of the interview to meet with struggling citizens and highlight the plight of the middle class.

“I think that Hillary has been to this rodeo a bunch of times. She is in public service because she cares about the same folks that I talked to here today. As soon as you jump back into the spotlight in a more explicitly political way, you’re going to be fly-spec like this,” Obama said.

“She’s accustomed to it,” he said. “Anybody who gets involved in public life is accustomed to it. Over time I don’t think it’s going to make a big difference.”

Though Clinton has faced many critics from the left and right following her remarks, Obama wasn’t the first to come to her defense. Clinton’s husband last week said wealth shouldn’t be the metric for one’s ability to sympathize with struggling Americans.

“I think I had the lowest net worth of any American president in the 20th century when I took office. But I still could have been tone deaf,” he said last week. “The real issue is, if you’ve been fortunate enough to be successful, are you now out of touch and insensitive to the agonizing struggles other people are facing?”

Clinton later said she was grateful for her husband’s support, but doesn’t need his defense, also saying her record “speaks for itself.”

We Can't Get Over How Great The 'Orange Is The New Black' Gay Pride Parade Float Is
